Thin-C
The novel corneal deswelling medium THIN-C, expressly designed to prepare corneal lamellar tissue for DSAEK/DALK by microkeratome and femtolaser, reduces significantly the donor corneal thickness before cut.
Four hour deswelling treatment of donor cornea in THIN-C before microkeratome cut resulted in significantly thinner posterior lamellae for DSAEK with respect to control corneas.
Twenty-one eyes of 21 patients with endothelial dysfunction due to Fuchs’ endothelial dystrophy and pseudophakic bullous keratopathy received the tissues deswelled in THIN-C at S. Giovanni-Addolorata-Britannico Hospital, Rome, Italy.
AS-OCT (Visante™, Carl Zeiss Medic Inc., Dublin, CA, USA) equipped with a custom-made FLEB OCT adaptor (Ophthalmic Biophysics Center, Miami, FL, USA) to carry out OCT measurements through the optically clear vial bottom, and confocal microscopy were used to evaluate corneal thickness, endothelial cell density, and morphology before transplantation and then at 7 days, 1 and 6 months after transplantation. A 13% and 30% decrease in corneal thickness was observed at 7 days and after 6 months, respectively. Endothelial cell density decreased on average by 43% after 6 months.
The opportunity to reduce significantly the donor corneal thickness, using THIN-C offers the advantage to prepare very thin lenticules for DSAEK, which contribute to better patient visual recovery.

Donor cornea stored in Eusol-C at 4°C for 7 days showed a corneal thickness of 670 μm when measured by OCT Visante equipped with a custom-made FLEB OCT adaptor to carry out measurements through the optically clear vial bottom.
